1-2A Violation of Code: [511-6-1.03(2)] Observed no PIC in the facility at time of inspection. There must be a person in charge on the premises of the food service establishment at all times. PIC arrived 30 minutes into inspection.  Corrected On-Site.  New Violation.
2-2D Violation of Code: [511-6-1.06(2)(c)] Observed three handsink in the facility without hot water (temperature reached 80F). A handwashing sink shall be equipped to provide tempered water at a temperature of at least 100ºF (38ºC) through a mixing valve or combination faucet. Pf PIC shall temporary close until hand sinks are able to produce hot water.  New Violation. Correct By: 03/30/2024
2-2D Violation of Code: [511-6-1.06(2)(o)] Observed rubber mats, coffee cups, food buildup, and plates in three hand sinks. A handwashing facility may not be used for purposes other than handwashing. Pf PIC shall educate employees and staff on proper handwashing sink usage.  New Violation. Correct By: 03/23/2024
4-2A Violation of Code: [511-6-1.04(4)(c)1(iv)] Observed fried egg, bacon, cheese being stored uncovered in a reach in cooler. Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. PIC provided lids.  Corrected On-Site.  New Violation.
4-2B Violation of Code: [511-6-1.05(7)(b)] Observed a heavy buildup of a mold like substance in the bulk ice machine. Except when dry cleaning methods are used as specified under subsection (7)(e) of this Rule, surfaces of utensils and equipment contacting food that is not time/temperature control for safety food shall be cleaned: (i) At any time when contamination may have occurred; (iv) In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment. PIC shall have the bulk ice machine cleaned.  New Violation. Correct By: 03/23/2024
4-2B Violation of Code: [511-6-1.05(7)(c)] Observed buildup on the interior walls of a microwave. The food-contact surfaces of cooking and baking equipment shall be cleaned at least every 24 hours. This subsection does not apply to hot oil cooking and filtering equipment if it is cleaned as specified under subsection (7)(b)3(v) of this Rule. PIC shall have the microwave cleaned.  New Violation. 4-2B Violation of Code: [511-6-1.05(6)(n)] Observed the dishwasher in the dish area failing to reach 50ppm chlorine. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions P PIC shall have the dishwasher repaired.  New Violation. 6-1A Violation of Code: [511-6-1.04(6)(f)] Observed TCS food items being held in cooling units above 41F (see chart below). Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below. PIC had TCS food items discarded.  Corrected On-Site.  New Violation.
6-2 Violation of Code: [511-6-1.04(6)(g)] Observed TCS food items (sausage, fried egg) without a date marking sticker. Except when packaging food using a reduced oxygen packaging method, and except as specified in paragraphs 4 and 5 of this subsection, refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days. The day of preparation shall be counted as Day 1. Pf PIC had TCS food items discarded.  Corrected On-Site.  New Violation.
12B Violation of Code: [511-6-1.03(5)(i)] Observed an employee preparing a panini for a customer without a hair restraint. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. PIC provided a hair restraint.  Corrected On-Site.  New Violation.
12C Violation of Code: [511-6-1.04(4)(m)] Observed a wet wiping cloth being stored on a door handle in the front service area. Cloths in-use for wiping counters and other equipment surfaces shall be: (i) Held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n); and (ii) Laundered daily. 14B Violation of Code: [511-6-1.05(10)(e)1,2,4] Observed a cardboard box of towels being stored on the floor underneath a table. Observed cardboard boxes of an end table and toxic chemicals being stored on the floor in a back room. Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (i) In a clean, dry location; (ii) Where they are not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 centimeters) above the floor. PIC moved the boxes.  Corrected On-Site.  New Violation.
15A Violation of Code: [511-6-1.05(6)(a)] Observed a broken door to dry storage in the front service area. Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. PIC shall have the door repaired.  New Violation. Correct By: 03/30/2024
15A Violation of Code: [511-6-1.05(6)(a)] Observed a reach in cooler in the front service area being held above 41F(46F). PIC had TCS food discarded stored in the reach in cooler discarded. Equipment shall be maintained in a state of repair and condition that meets the requirements. PIC shall have the reach in cooler repaired.  New Violation.
15C Violation of Code: [511-6-1.05(7)(d)] Observed heavy buildup of food and debris in the reach in cooler/freezer walls, shelving, and doors in a back storage room. Observed buildup on the shelving above the panini press in the front service area. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. PIC shall have the non food contact surfaces cleaned.  New Violation. 16A Violation of Code: [511-6-1.06(1)(g),(h)] Observed hand sinks, prep sinks, three compartment sink failing to reach a minimum of 100/110F (79/80F). Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food service establishment. Pf PIC shall temporary close until hot water is restored to the facility.  New Violation. Correct By: 03/30/2024

17C Violation of Code: [511-6-1.07(5)(f)] Observed the mop head being stored in the wringer of a mop bucket near the mop sink. After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. PIC had the mop head stored to air dry.  Corrected On-Site.  New Violation.
17C Violation of Code: [511-6-1.07(2)(a)] Observed porous ceiling tiles in a storage area. Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. PIC shall have ceiling tiles replaced.  New Violation. Correct By: 03/30/2024
18 Violation of Code: [511-6-1.07(5)(k)] Observed a cardboard box underneath a front service area sink with evidence of pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies; 2. Routinely inspecting the premises for evidence of pests; 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and 4.
