OBSERVATIONS AND CORRECTIVE ACTIONS
4-2A Violation of Code: [511-6-1.04(4)(c)1(iv)] Observed at time of this inspection multiple foods stored uncovered in reach in freezer next to fryer. COS: Manager had employees put lids on the containers. CA: Manager stated she would ensure foods remain covered to prevent contamination.  Repeat Violation.
6-1A Violation of Code: [511-6-1.04(6)(f)] Observed at time of this inspection the prep top cooler in the grill area was holding TCS foods well above the required 41F. COS: TCS Items were removed due to its inability to hold temperature for food safety. CA: Dispose of TCS foods in prep top cooler and have cooler repaired or replaced.  Repeat Violation.
6-2 Violation of Code: [511-6-1.04(6)(g)] Observed 1 container each of onions, lettuce, and sliced tomatoes with no proper date marking. Once veggies are sliced, you must have prep date or discard date on container.  New Violation.
13A Violation of Code: [511-6-1.02(1)(d)] Observed the last inspection not being posted. CS: Keep most up to date inspection posted for public view.  New Violation.
