OBSERVATIONS AND CORRECTIVE ACTIONS
2-2A Violation of Code: [511-6-1.03(4)(a)(b)(c)(e)(f)] Management unable to provide employee health policy with all necessary items (signed by employees, exclusion/restriction, etc) for a selected employee, the dish washer. CA: All food service facilities must have an employee health policy where food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food.  New Violation.
2-2B Violation of Code: [511-6-1.03(5)(j)1&2] Observed employee drinks in an improper storage area, missing the required disposable cup, lid and straw. CA: Employees shall consume food/drink only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed. COS: Drinks discarded.  Corrected On-Site.  New Violation.
4-2A Violation of Code: [511-6-1.04(4)(c)1(iv)] Observed ice bin stored uncovered in food service area. CA: Except when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. COS: Ice bin was covered.  Corrected On-Site.  New Violation.
5-1B Violation of Code: [511-6-1.04(5)(h)] Observed refried beans in the steam well at time of service at 141 deg, not properly reaching required 165 deg F. CA: Reheating for hot holding shall be done rapidly and the time the food is between the temperatures of 41°F (5°C) and 165°F (74°C) and 41°F (5°C) and 135°F (57°C) for commercially processed food, may not exceed 2 hours. COS: Beans reheated to above 165 deg F.  Corrected On-Site.  New Violation.
6-1A Violation of Code: [511-6-1.04(6)(f)] Observed time/temperature control for safety (TCS) foods in the walk-in cooler at 56 and 61 deg. TCS foods must remain at 41°F or below for cold-holding. Person in charge instructed employee to begin the reheat process to reheat TCS to 165°F.  Repeat Violation.
11A Violation of Code: [511-6-1.04(6)(a)] Observed the freezer for the make line not properly holding food items frozen. CA: All freezer units must hold foods frozen solid, the air temperature shall not be higher than 10 deg F.  New Violation.
12C Violation of Code: [511-6-1.04(4)(m)] Observed the wiping cloth sanitizer for the bar weak at 10 ppm. CA: Cloths in-use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n). COS: Moved to 50 ppm sanitizer.  New Violation.
14A Violation of Code: [511-6-1.04(4)(k)] Observed scoop handle stored in direct contact with food. CA: In food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon. COS: Handle moved out of food.  New Violation.
