Observed employee wash hands in 3 compartment sink by dipping hands in detergent solution in wash compartment and then turn off faucet without using a paper towel. Observed second employee wash hands in 3 compartment sink. New Violation.

In food preparation areas employees must use a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles New Violation.

Observed significant build-up of food residue on pizza screens. Manager stated these can't be washed and are run through the oven to burn the food debris off of them. Observed build-up of food residue inside cheese container in prep-top. New Violation.

Observed vent fan not working in restroom. This is a violation on the previous inspection. Observed accumulation of food debris under prep tables, behind ovens, under dry storage shelves, behind chest freezer, and behind mixer. Observed sagging ceiling tiles in kitchen. These should be replace with smooth, non-porous tiles that are faced with a durable, smooth, cleanable material. Observed damage to walls throughout Repeat Violation.
