2-2A Violation of Code: [511-6-1.03(4)(a)(b)(c)(e)(f)] Observed PIC could not provide documentation or otherwise satisfactorily demonstrate during the inspection that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. According to Code: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. A copy was given to PIC and instructed to train employees on the employee health policy and keep documentation of training onsite  Repeat Violation.
4-2B Violation of Code: [511-6-1.05(7)(b)] Observed mildew like/biological growth build up on deflector plate in ice machine. [511-6-1.05(7)(a)1]
C/A: Informed PIC to remove deflector plate and clean free from observed mildew/biological growth build up.
Advised PIC to increase the frequency of cleaning ice machine to prevent the accumulation of mildew like growth.  Repeat Violation.
6-1A Violation of Code: [511-6-1.04(6)(f)] Observed time control for safety / potentially hazardous food cold held at greater than 41 degrees Fahrenheit. Observed chicken in RIC at 50 F. Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. PIC moved high temperature TCS food Items into a walk-in cooler.  New Violation.

(g) Ready-to-Eat Time/Temperature Control for Safety Food, Date Marking

1. Refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days. The day of preparation shall be counted as Day 1.  New Violation.
12B Violation of Code: [511-6-1.03(5)(i)] Observed employee (PIC) not wearing a hair restraint. 511-6-1.03(5)(i) - Hair Restraints (C)
(i) Hair Restraints.

1. Employees preparing or handling food shall use effective and clean, disposable or easily
cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain
loose hair including beards and mustaches longer than one half inch.

2. This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles.  New Violation.
15C Violation of Code: [511-6-1.05(7)(d)] Observed non food contact surface of the facility wall, floor ceiling and equipment not clean to sight and touch. Floor, wall and equipment are so sticky to walk on. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.  Repeat Violation.
17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed floor, ceiling and walls not maintained. There are holes on the wall and floor, ceiling and walls decade and worn out. Observed unnecessary items/litter crowding storage area. The premises shall be free of items that are unnecessary to the operation or maintenance of the establishment, such as litter or equipment that is nonfunctional or no longer used and litter. PIC advised to clean up and dispose of unnecessary items. All physical facilities shall be maintained in good repair.PIC was given 60 days to get the whole facility repaired.  Repeat Violation.

(k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

1. Routinely inspecting incoming shipments of food and supplies;

2. Routinely inspecting the premises for evidence of pests;

3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and

4. Eliminating harborage conditions. PIC could not provide pest control invoice during inspection.  New Violation.
Remarks I spent 20 minutes educating PIC (Latia) and the owner (Bob).
