2-2D Violation of Code: [511-6-1.06(2)(o)] Observed dirty food contact utensils stored in the hand washing sink. Advised PIC that hand washing sinks may only be used for hand washing purposes. COS: Items removed from sink.  Corrected On-Site.  New Violation.
6-1D Violation of Code: [511-6-1.04(6)(i)] Observed PIC using Time as a Public Health Control (TPHC) for sushi rice. Observed an employee writing in sushi rice times for 12/19/16 and today. Cooked sushi rice observed at 98F. Advised PIC that when the food is initially put out it should go out at the correct temperature and the employee should record the time that the item was put out- discard time. Total time should not exceed 4 hours.  Corrected On-Site.  New Violation.
8-2B Violation of Code: [511-6-1.07(6)(g)] Observed chemical sanitizing solution , that is applied to food contact surfaces, reading above 100 ppm cl-. COS: Solutions replaced 50 ppm cl-.  Corrected On-Site.  New Violation.
8-2B Violation of Code: [511-6-1.07(6)(c)] Observed multiple containers of bleach stored above packages of noodles. COS: PIC relocated containers of bleach.  New Violation.
17A Violation of Code: [511-6-1.07(5)(i)] Door that leads from restroom into dining area is not tight fitting or self closing. Advised PIC to repair the door so that it is tight fitting and self closing.  Repeat Violation.
Remarks Dish Machine: 50 ppm cl-
Observed verbal/written employee health policy, written procedures for fecal/vomit clean-up, food invoices, and parasite destruction letters.
Observed one can of dented food. PIC separated container from other food products and labeled it to be returned to sender. Majority of cans in compliance.
